症例は 73歳男性。膵尾部腫瘤に対し経胃的に超音波内視鏡下穿刺吸引（EUS-FNA）が施行され，病理組織学的に腺癌の診断となった。術前化学療法の後に膵体尾部切除術が施行され，最終病理診断は浸潤性膵管癌，pT3N0M0であった。その後経過観察されていたがCA19-9の上昇を認め（38.7 U/mL），CTで胃壁の肥厚が出現した。FDG-PET CTが施行され，胃壁にSUVmax 10.1の集積を認めた。そこで，上部消化管内視鏡検査を施行され，胃体上部から体中部小弯後壁に径20mmの粘膜下病変が認められた。生検結果で膵癌の組織と同様の腺癌であり，穿刺経路腫瘍細胞播種（needle tract seeding : NTS）による胃壁への膵癌再発の診断となった。EUS-FNAでの胃壁穿刺部が手術後も残る膵体部癌・膵尾部癌では，穿刺部位への再発がないか慎重な経過観察が必要である。 |

A 73-years-old male underwent transgastric Endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) for a pancreatic tail tumor. Histopathological examination evaluated the tumor as adenocarcinoma. The patient received preoperative chemotherapy and subsequently underwent distal pancreatectomy. Final pathological diagnosis was invasive pancreatic ductal carcinoma, staged as pT3N0M0. Follow-up evaluations showed increases in CA19-9 to 38.7 U/mL and thickening of the gastric wall on computed tomography (CT) images. Further investigation with 18F-fluorodeoxyglucose positron emission tomography CT (FDG-PET CT) showed a maximum standardized uptake value (SUVmax) of 10.1 in the thickening gastric wall. Upper gastrointestinal endoscopy identified a 20 mm submucosal lesion extending from the upper part of the stomach to the posterior wall of the lesser curvature in the middle part of the stomach. A biopsy of the lesion showed adenocarcinoma, which is consistent with gastric wall recurrence of pancreatic cancer due to needle tract seeding. It is important to carefully observe patients with pancreatic body and tail cancer who have undergone EUS-FNA and have residual puncture sites after surgery to detect recurrence at the puncture site. |
