| 内容記述 | 症例は62歳，男性。右下腹部痛を主訴に来院した。Computed tomography（以下 CT）所見で回盲部腫瘤を認めた。術前の血液検査で血清CA19-9が351.5 U/mlと高値を示し，悪性腫瘍を疑い腹腔鏡下手術に臨んだ。腫瘤は虫垂に癒着しており，虫垂と共に摘出した。摘出後回腸末端から全小腸を確認するとTreitz靱帯から約200cmの位置から起始している約10cmの重複腸管を認め，回腸部分切除を行なった。病理検査の結果，管状部分は腸管の全層構造を有する小腸組織で構成され，先に摘出した腫瘤との間に連続性が確認された。管状かつ球状嚢胞の形態を呈する消化管重複症と診断されたが悪性所見は認められなかった。成人発見の重複腸管は比較的稀で，本邦の報告では術前腫瘍マーカー高値を示した症例の多くは悪性腫瘍であったが，本症例では当てはまらず，形態と合わせて非常に稀な症例を経験したので若干の文献的考察を加えて報告する。 | |||||||||||||||||||||||||||||||||

| 内容記述 | A 62-yearold man presented with pain in his lower right abdomen. Computed tomography (CT) revealed a cystic lesion in the ileocecal region. A preoperative blood examination indicated an elevated level of serum CA19-9 （351.5 U/mL）. The possibility of a malignant tumor in the ileocecal region prompted laparoscopic surgery, which confirmed the presence of a cyst attached to the appendix and a 10 cm ileal duplication located 200 cm from the Treitz ligament. Subsequently, a partial resection of the ileum was performed. Pathological examination of the specimen revealed that the ileal duplication consisted of ileal tissue, exhibiting a full-thickness structure resembling the intestinal tract. Moreover, a connection was established between the cyst and the ileal duplication, confirming their continuity. The patient was diagnosed with a tubular and cystic gastrointestinal duplication without any malignant characteristics. Tubular and cystic gastrointestinal duplications are rare conditions, often accompanied by elevated preoperative tumor markers. The absence of malignancy in such cases is even rarer, prompting this case report alongside a comprehensive review of the existing literature. | |||||||||||||||||||||||||||||||||
