| 内容記述 | 症例は58歳男性，繰り返す右下腹部痛を主訴に当科を受診し，CTで認めた虫垂内部の糞石による症状と考え，腹腔鏡下虫垂切除術を施行した。術後病理検査で虫垂神経内分泌腫瘍（Neuroendocrine tumor:以下，NET）を認めたが，再発や転移のリスク因子はなく経過観察の方針とした。虫垂NETは稀な腫瘍で，後病理検査で偶発的に発見されることが多い。虫垂に発生する神経内分泌腫瘍は，本邦の取扱い規約においては組織学的異型度によって分類されるが，WHO分類では細胞増殖能による分類がなされており，本邦で発刊されている神経内分泌腫瘍のガイドラインではWHO分類に則った推奨術式のプロトコルが提示されている。今症例の検討にあたり， WHO分類の変遷に関して最新版のWHO第5版も参照して振り返りつつ，本邦での報告例について検討したため，ここに報告する。 | |||||||||||||

| 内容記述 | We report a case of appendiceal neuroendocrine tumor (NET). A 58-year-old man came to our hospital complaining of recurrent right lower abdominal pain. After series of clinical examinations, we considered this symptom to be caused by fecal stone in appendix, thereafter, we performed laparoscopic appendectomy. Pathological examination revealed an appendiceal NET in the body of the appendix, but there were no risk factors for recurrence or metastasis, and we decided to follow up the patient without adjuvant therapy. Appendiceal NETs are rare tumors and are often discovered incidentally on posterior pathological examination. In Japan, neuroendocrine tumors of the appendix are classified according to histological atypia, whereas the WHO classification is based on cell proliferative capacity. And Japanese guidelines for neuroendocrine tumors provide surgical protocols in accordance with the WHO classification. In reviewing this case, we referred to the latest 5th edition of the WHO classification and reviewed the case reported in Japan. | |||||||||||||
