| 内容記述 | がん看護に携わっている看護師が,マッサージを患者に実践していくことで,がん患者がかかえている痛みへの理解が全人的なものへと深まる過程を明らかにすることを目的に,看護師に半構成的な面接を行った.その結果面接で看護師たちが語った内容は,がん患者が抱えている痛みに対する自分の在りようやがん患者その人に対する自分の在りようであった.看護師は患者にかかわる前,【身体的な痛みの緩和を薬に頼ろうとする私】であり,がん患者の痛みを【家族の痛み】と捉え,自分の在りようは【痛みを持つ患者との関係性の中で,関心が自分に向いている私】,【痛みを持つがん患者にかかわる上で自己の能力を高めたい私】であった.マッサージを用いた看護実践を経て,看護師は【がん患者とかかわる自分に向き合えなかった私】,【痛みを持つがん患者に向き合っていなかった私】である自分の在りように気付き受け止め,【痛みを持つがん患者に向き合おうとする私】へと変化した.また,患者とのかかわりの知に成熟した看護師と共に臨床実践の場でかかわり{自己を振り返る経験}を重ねることが,自己知を高めるために必要なサポートとして有効であることが示唆された. | |||||||||||

| 内容記述 | The aim of this study was to clarify the process by which cancer care nurses deepen their understanding of patients' pain from a partial to holistic one. We conducted semi-structured interviews with nurses and observed the interaction between the nurses and patients during massage care. During the initial interviews, the nurses expressed their thoughts and feelings toward patients' pain and toward cancer patients themselves. Before actual interaction with patients, the nurses "wanted to depend on medicine for physical relaxation" and the nurses regarded patients' pain as "the pain suffered by their family". They viewed themselves as "nurses who are concerned only about their skills and techniques when facing patients in pain" and as "nurses who want to improve their skills through experiencing relationships with patients in pain". Through their experience in massage as a nursing practice, the nurses became conscious of their inner feelings, such as "I am unable to accept and face my inner self that interacts with cancer patients" or "I am not trying to face cancer patients who are in pain". Their state of mind also started to change into a feeling such as "I am trying to face cancer patients who are in pain". This study also indicates that the following are effective for supporting nurses in deepening their understanding of their feelings and inner selves: using massage care as a technique for interacting closely with patients; working closely with experienced nurses who have knowledge, understanding, and wisdom regarding interaction with patients and who can share their experiences with others; and repeating the experience and process of self-reflection. | |||||||||||
