| 内容記述 | 症例は60歳の女性。自家用車で自損事故を起こし，その時の記憶がなく近医を受診した。38°C台の発熱と意識障害があり，MRI画像にて右側頭葉内側病変と島皮質病変を認め，単純ヘルペス脳炎の疑いでアシクロビルの投与が開始された。後日，精査加療目的に当院に転院した。当院で施行した髄液検査で細胞数増多，単純ヘルペスDNA PCR陽性でヘルペス脳炎と診断した。アシクロビル投薬を継続し，後遺症なく症状改善した。前医で投薬が早期に開始されていたことで，合併症は認めず予後良好であったと考えられる。臨床症状と頭部MRI検査で単純ヘルペス脳炎が疑われる場合には早期にアシクロビルを投与すべきと考えられた。 | |||||||||||||||||

| 内容記述 | The 60 year-old woman had a fever of 38°C and impaired consciousness, and MRI images showed a medial lesion in the right temporal lobe and an insular lesion in the insular cortex. This information is estimated herpes simplex encephalitis, and acyclovir treatment was started. The patient was transferred to our hospital, and the cerebrospinal fluid test performed at our hospital showed increased cell counts and positive herpes simplex DNA PCR. We diagnosed herpes encephalitis, so medication was continued, and the symptoms improved without sequelae. Because of the medication was started early by the previous hospital, there were no complications. We learned that if herpes simplex encephalitis is suspected by clinical symptoms and head MRI images, acyclovir treatment should be started as soon as possible. | |||||||||||||||||
