| 内容記述 | 小建中湯は，主に虚弱児の腹痛などの消化器症状に使用される。今回消化器症状と異なる症状を呈した姉弟が小建中湯で良好な結果を得たので報告する。症例1は癇癪，嘔吐反射，鼻出血が主訴の4歳男児である。抑肝散では効果が不明で，小建中湯に変更したところ，主訴に加え皮膚疾患も改善した。症例2は下肢痛で歩行不可の7歳女児である。器質的問題はなく3か月のリハビリでも改善ないため小建中湯を与えたところ，3週間で歩行可能となり5週間で完治した。漢方薬は証に従い治療を行う随証治療で，異なる症状でも証が合えば同様の漢方薬が適応となる異病同治を行う。古典の条文から小建中湯の使用目標は，腹部の筋肉が突っ張り痛む，動悸，鼻出血，手足疼痛などとされ，要点は脾胃虚（消化器機能の虚弱）である。虚弱の判定が困難なときは，便秘などの胃腸障害，細い体形，また，腹壁の緊張，四肢冷感，擽感がみられるなどの所見を参考に小建中湯を考慮し，脾胃虚に端を発した様々な症状に効果が期待できる。症例1の初回に処方した抑肝散は肝気が亢進し頭に血が上って憤怒するが，しかし顔面，身体は蒼白であり，腹部は虚弱で怒りが性急である症例に適応がある。構成生薬は，白朮，茯苓，甘草で体力や胃腸機能を補い（補気），当帰，川芎で補血し，柴胡，釣藤鈎で肝気旺盛を収めると理解でき，症例1の症状に合致する。一方，小建中湯は桂皮が気をめぐらせ，芍薬が補血，腹部の鎮痙作用，生姜が消化機能を助け，膠飴で元気をつける処方である。症例1は脾胃強弱であり，まず脾胃を補う処方（小建中湯）が肝要なのではないかと考えた。また，一見虚証に見えない症例2 では腹直筋攣急や擽感から虚証と捉え，腹部の状態を立て直し元気をつける建中湯（小建中湯）を服用することで四肢疼痛が改善したと思われた。小建中湯は虚証や脾胃虚に端を発した虚弱児の症状の改善に寄与できる可能性がある。 | |||||||||||||

| 内容記述 | Shokenchuto is a Kampo formula that is used for abdominal pain in children with poor gastrointestinal health. We here present two cases of siblings treated with shokenchuto for non-gastrointestinal symptoms. Case 1 was a 4-year-old boy with tantrums, gag reflex, and epistaxis, as well as skin rash on the limbs, all of which improved after treatment with shokenchuto. Case 2 was a 7-year-old girl who was unable to walk due to lower limb pain. On examination, no abnormalities in the bones or nerves were observed, and her symptoms did not improve after 3 months of rehabilitation ; however, she completely recovered after 5 weeks of taking shokenchuto. Kampo medicine is a type of treatment that is performed according to a pattern, which refers to diagnosis obtained through traditional Japanese pathophysiological assessment, and if the pattern matches, the same Kampo formula is indicated regardless of different symptoms. The classic text of Kampo medicine, Shang Han Lun, describes that the target symptoms of shokenchuto are nosebleeds as well as pain in the abdominal muscles and extremities, and it also aims to resolve any deficiencies in the spleen and stomach (TM) (poor gastrointestinal health). Case 1 had spleen and stomach (TM) weakness ; thus, shokenchuto was required to supplement the functions of these organs. In addition, in Case 2, which did not appear to have a deficiency pattern on initial examination, the patient’s rectus abdominis muscle spasms and skin sensitivity were interpreted as being a deficiency pattern, and use of shokenchuto relieved the foot pain. In conclusion, shokenchuto can be effective for various symptoms of weak pattern originating from deficiencies in the spleen and/or stomach (TM). | |||||||||||||
