| 内容記述 | 近年になり高齢者の増加に伴って骨粗鬆症性骨折患者が増加している。骨粗鬆症性骨折は，介護が必要になる主な要因の一つで健康寿命を損なうばかりでなく生命予後にも悪い影響を与え，医療経済的損失が大きいと言われている。骨折予防の取り組みとしては骨粗鬆症検診がある。しかし，検診受診率の全国平均が5.5%（2022年度）と極端に低く，骨折患者の殆どが骨粗鬆症の診療を受けることなく骨折している実態がある。社会全体で骨粗鬆症性骨折を予防し健康寿命を延伸させるための骨粗鬆症リエゾンサービスOsteoporosis Liaison Service（OLS）を確立させることが急務である。そこで，福島市におけるOLSを確立するために，自治体，医師会，そして医療機関の協力を得て骨粗鬆症検診制度の改革を行った。その結果，受診率が向上し，骨粗鬆症予備群であるハイリスク者の選別が可能となり，精密検査で骨粗鬆症と診断された者に対しては即座に治療を開始することができるようになった。さらに検診で診断された骨粗鬆症患者の地域連携診療を推進するために各医療機関の診療意向や治療可能薬を表示できる骨粗鬆症診療マップを作成し公表した。骨粗鬆症に対する治療の目的は骨折予防である。従って，一旦治療が開始されれば将来に渡って治療の継続が不可欠であるため，高齢者の場合はかかりつけ医において生活習慣病とともに骨粗鬆症診療を継続し，骨折予防を達成する必要がある。この診療マップを活用することで適正な患者紹介が可能となり，連携診療が活性化してきた。また，骨粗鬆症性骨折を予防するためには地域住民の骨の健康に関する認識を高める必要がある。そのため骨粗鬆症教室を開催するなど啓発活動に取り組んできた。本稿では，骨粗鬆症性骨折の実態，骨粗鬆症検診から骨折予防を達成するための一連の取り組みの内容を概説した。 | |||||||||

| 内容記述 | In recent years, the number of patients with osteoporotic fractures has increased in the elderly people. The reality is that most fracture patients suffer fractures without ever receiving treatment for osteoporosis. There is an urgent need to establish osteoporosis liaison services (OLS) in the local community. Therefore, to establish OLS in Fukushima City, we reformed the osteoporosis screening system with the cooperation of the local government, the medical association, and medical institutions. As a result, the rate of people undergoing osteoporosis screening has increased. It has become possible to select those at high risk for osteoporosis. Furthermore, it has become possible to immediately start treatment for those who are diagnosed with osteoporosis through detailed examinations. An osteoporosis treatment map was created to promote regional collaborative medical care for osteoporosis patients. This map was created to display medical institutions that can treat osteoporosis. By using this map, regional collaborative treatment for osteoporosis patients has been revitalized. To prevent osteoporotic fractures, it is necessary to raise awareness among residents about bone health. We have been working on awareness-raising activities, such as holding seminars on osteoporosis. This paper describes the current state of osteoporotic fractures and details our series of efforts to achieve fracture prevention through osteoporosis screening. | |||||||||
