Therefore, in order to change the organization to provide safe medical care, a “reporting culture” must be created in which employees voluntarily report errors and near misses. We have introduced an incident reporting system and increased the feedback of medical safety information to all staff with the aim of increasing the number of incident reports submitted. Furthermore, the type of information being provided has diversified, including not only incident cases that occurred within our facility, but also cases of other facilities reported in the mass media, Good Job cases, and re-notification of hospital rules. As a result, the number of incident reports submitted increased 14.0 times to 9,077 in 2023 compared to 649 incident reports submitted in 2014, before starting the initiative.
