本研究の目的は，がん患者とその家族に日常的に接している看護師が，がん患者・家族の悩みの相談を受けることで，患者や家族が悩みや問題を解決できるように導く能力を開発する教育プログラムの構築を図り，その成果を評価することである．困難事例の事前調査では，がん患者の心理的ケア，家族看護，スピリチュアルケアの難しさが明らかとなった．これらの内容を取り入れ２年間継続して行う教育プログラムを検討した．１年目は，臨床経験５年以上の看護師22人を対象に，悩み相談での患者・家族との相互作用やコミュニケーションスキルの学習，相談を受ける能力の訓練としてロールプレイを取り入れ，その成果を日常の看護実践に活かした．ワークショップで看護態度や看護行為を振り返ったことで，相互作用の理解が深まるとともに，それぞれの行動変容を認めることができた．２年目は，患者と家族の悩みや心配の相談の実践を行い，相互作用と傾聴と沈黙などのコミュニケーションスキルを振り返った．２年間の教育プログラムに参画した看護師は12名だった．２年間の教育の成果として，患者や家族と良好なコミュニケーションをするために環境に配慮し，非言語的コミュニケーションやパーソナリティに注意深く対処することができるようになり，自分自身の看護を肯定的に評価し，今後に向けた課題が明確になっていた． |

The purpose of this study was to establish a skill development program that enables nurses who daily attended cancer patients and received complaints and grievances of cancer patients and their families so that they would be able to help the patients or families solve those issues. A preliminary survey found the difficulty of family care, psychological care of cancer patients, and the spiritual care. We planned a sustained 2-year educational program that addressed the above issues. In the first year, 22 nurses with 5-year or more clinical experience learned about interaction with and among cancer patients and their families, and practiced communication skills. The nurses who participated in the workshop were expected to apply the skills gained from the role-playing training in daily nursing works. Reflecting on their own nursing attitudes and behaviors, they could develop clear future goals and modify their own behaviors. As a result, deep mutual understanding was achieved. In the second year, they actually served worried patients and family members, and reviewed receptive interaction and other communication skills including attentive listening and silence. 12 nurses continued to participate in the educational program for the entire two years. They scrutinized the overall process of trust-building, interaction and spiritual care. The learning program devised in this study encouraged nurses to be more aware of the environment for good communication, non-verbal communication, and personality issues. The participating nurses also evaluated their own nursing practice more positively and articulated future challenges more clearly. |
